CONTRACT COURSE ACADEMIC CREDIT REGISTRATION
INFORMATION AND REGISTRATION FORM

PLEASE READ ENTIRELY:

I. REGISTRATION REQUIREMENTS:

1. This registration form is only to be used by students participating in special "contract" courses and/or programs that are facilitated by the Office of Continuing Education for either graduate or undergraduate credit. You should use this form only if you have been instructed to do so by your instructor or program/workshop coordinator. Also, you must complete all requirements specific to your course registration.

2. Graduate credit registration requires that you provide proof of highest degree received if you have never been enrolled in the Graduate School at OSU. New graduate credit students will be admitted as nondegree status (refer to the nondegree status information below) unless they have been admitted to a graduate program for the current quarter. Please disregard the proof of degree requirement if you have previous enrollment in the Graduate School or if you have recently been admitted to a graduate program and have submitted transcripts to the Graduate School.

3. Ohio State University English Proficiency Requirements: all applicants (new students) who's native language is not English, and who have been educated primarily outside of the U.S., must submit official TOEFL (Test of English as a Foreign Language), MELAB (Michigan English Language Assessment Battery) or IELTS (International English Language Testing System) scores. The scores must be sent directly from the testing agency to Ohio State University. The acceptable scores are as follows: Paper Based TOEFL (PBT) – 550; Computer Based TOEFL (CBT) – 213; Internet Based TOEFL (IBT) – 79; MELAB – 82; IELTS – 7.0. TOEFL information can be found at www.toefl.org, MELAB information can be found at www.lsa.umich.edu/eli/melab.htm, and IELTS information can be found at www.ielts.org.

Applicants/students who are citizens of or who have received a bachelor’s degree or higher in one of the following countries are exempt from the English Proficiency Requirement: Australia, Belize, the British Caribbean and British West Indies, Canada (except Quebec), England, Guyana, Ireland, Liberia, New Zealand, Scotland, the United States, and Wales.

Questions regarding Ohio State University English Proficiency Requirements should be directed to any counselor in the Graduate/International Admissions Office at 614-292-9444 or international.grad@osu.edu.

4. OSU e-mail account and OSU Internet Username activation: all students should activate their OSU e-mail account and OSU Internet Username. Activation information is available at www.oit.ohio-state.edu or by calling the Help Desk at 614-688-HELP(4357).  Please wait 2 days after your registration has been processed before requesting activation.

4a.  Online/web-based course access: Ohio State uses the "Carmen" online/web course management system. Students can access online/web-based courses through the Carmen web site at http://telr.osu.edu/carmen (OSU e-mail account and Internet Username activation is required.). Please note that courses usually are not posted/activated in the Carmen system until the first day of the class or 1-2 days prior and therefore the information is not available until that time.

5. Personal Data Information Release: per a student’s designation, the University may or may not release certain information about each student to the public or through the student directory. Refer to the privacy act, directory information section on the Registrar’s Office web site, www.ureg.ohio-state.edu for more information. After your enrollment registration has been processed, you may request the “no release” option by going to the Registrar’s Office web site, click on Online Forms under the Current Student tab and then select and fill out the Request for Change of Records form with your information release designation. Please review and understand the restrictions of the “no release” option before requesting the designation.

6. Student Fee Accounts: fees for contract courses are processed as a group and are not posted to student fee accounts in the same manner as regular course enrollment but will appear as PAID status on the student database. Please contact Richard Shafer, shafer.5@osu.edu, in the Office of Continuing Education (614-292-1664) if you have any questions regarding fee payment status for contract course enrollment.

7. Name change: if you were previously enrolled at Ohio State and your name has changed since your last enrollment at Ohio State, you must contact the Registrar’s Office if you want your current name listed on your student record.

8. Student Health Insurance: enrollment in contract courses is exempt from Ohio State Student Health Insurance coverage. Please contact the Student Health Insurance Office (614-688-7979) for information if you are not taking other courses that qualify you for insurance coverage and you are in need of short-term health insurance.

9. Registration cancellation: your registration will be cancelled if you do not submit all applicable registration materials and information (registration form, proof of degree, English proficiency, etc.). Also, your registration will be cancelled if you were previously enrolled at Ohio State and there is a HOLD on your student account. HOLDS must be cleared by calling the Office of Accounts Receivable (614-292-1056).

Graduate credit nondegree admission status: If you have never been enrolled in the Graduate School at Ohio State, you will be admitted as a Graduate nondegree student (please read the “Restrictions for Nondegree Student Status” section on the registration form). Also, proof of highest degree received (bachelor’s degree minimum) is required if you have never been enrolled in the Graduate School at Ohio State. Proof of degree must either be a photocopy of your diploma (preferred) or a copy of your transcript that clearly indicates your name, the college/university, degree/date received, and it must be from the institution issuing the degree. On-line/web transcripts and Teaching Certificates cannot be accepted as proof of degree. Proof of degree must be faxed or scanned and e-mailed to Richard Shafer, shafer.5@osu.edu, FAX: 614-292-0049. Please include the course or program name on the FAX Cover Sheet or in the e-mail message.

II. GRADES AND TRANSCRIPT INFORMATION:

Official/final grades usually are not available until after the designated quarter of enrollment has ended. Ohio State does not mail grades to students. Grades can be viewed online through the University Registrar’s web site at www.ureg.ohio-state.edu. Assistance for gaining web access and Ohio State student e-mail account activation is available by contacting the Ohio State Office of Information Technology online at www.oit.ohio-state.edu or the Help Desk at 614-688-HELP(4357).

Transcripts: Information for ordering transcripts is available on the University Registrar’s web site at www.ureg.ohio-state.edu (Office of the Registrar-Transcript Division, 320 Lincoln Tower, 1800 Cannon Dr., Columbus, OH 43210, 614-292-8500).

III. INSTRUCTIONS:

1. Complete and submit (electronically) the on-line Contract Course Registration Form. All required and other applicable fields on the form must be completed (Dept. & Course and/or call # may be omitted if unknown). After you have “clicked” to submit your form, your completed form will appear on the screen (a confirmation number will be indicated above your name). Please note: U.S. citizens must provide their Social Security Number on the registration form.

2. Print a copy of the completed form for your records.

3. FAX proof of degree (if applicable) to the attention of Richard Shafer at 614-292-0049. Include the course and/or program name on the fax cover sheet.

4. Please notify (e-mail) Richard Shafer, shafer.5@osu.edu, that you have submitted your registration. A confirmation reply will then be sent to you.
